Daily Screening

Please fill out the below information each day PRIOR to arriving at the daycare to help us expedite the drop-off process for all families. Entry can be denied until the form is completed.

If you answer 'Yes' to question #1, you must stay home & self-isolate, and get tested or contact a health care provider. If you answered ‘Yes’ to questions #2, 3, 4 or 5 , stay home and follow public health advice.
