Welcome Parents to Junction Daycare

For new families:

1. Please print off and fill out the forms

2. Sign all forms that indicate a signature

3. All forms will need to be filled out and returned to director@junctiondaycare.com and cc’d to admin@junctiondaycare.com

4. We will be in touch to go over the forms before your child starts care. Children cannot begin care until fees are paid and paperwork has been completed correctly. If you have any questions, please contact the Director.

Whenever visiting the daycare or if your child is ill, complete the Daily Screening form as a reference to know if your child should attend or not. No need to submit the form. It is as a reference only.
